Human Trafficking Victims: Request to Block Trafficking-Related Information

ClearStar is committed to supporting victims of human trafficking. If you find, or expect to find, negative information within your background check that results from your being a victim of human trafficking, you can request to have this information blocked from appearing in consumer reports we furnish about you. Note: This process is different from a standard background check dispute. It is a specific request to block trafficking-related adverse information under federal law. Therefore, we ask you submit your request via the webform on this page or by snail mail to ClearStar’s address, which is provided in the How to Submit Your Request section below.

What Information Must You Provide?

ClearStar requires the following information be submitted in any request to block trafficking-related information:

1. Proof of Identity

Please provide a scanned or photographed image of your driver’s license, passport, passport card, or other government-issued ID. A utility bill can be provided if you do not have the above documents.

2. Trafficking-Related Documentation

Please provide at least one of the documents below:

• A victim status determination from a governmental entity (state, federal, or tribal entities)

• A victim status determination from a non-governmental entity or a participant of a human trafficking task force authorized by the government to make victim status determinations

• A court document indicating victim status

• An attestation (signed statement) by you or your representative stating you are a human trafficking victim. This document must have an additional signature from a government entity, non-governmental organization, human trafficking task force, or a court representative

3. Identification of the Specific Negative Information You Wish to Block

This information must be related to your being a victim of human trafficking.

4. Your Preferred Contact Method

We will use this contact method in the event we need additional information from you.
